3. Application forms should be sent through ERAS (see Links).
4. Curriculum Vitae.
5. Personal Statement (be specific concerning your professional interest,

ie General Surgery, Subspecialty.

6. Official Transcript of credits fro Pre Medical Institution with GPA.
7. Letter from the Dean of the School of Medicine starting your academic rank in your class and moral qualities.
8. Official evidence of having approved the USMLE parts I and II or the Puerto Rico Board of Medical Examiners parts I and II.
9. Diploma of Medical School.
10. Fully bilingual – English/Spanish
11. Two letters of recommendation preferably from surgeons of Puerto Rico.

If Foreign Medical School Graduate:

1. Official ECFMG Certificate
2. Visa permit if not a U.S. citizen
